The answer is simple: refinements shouldn’t be the norm if the right principles are followed. Here’s what we’ve learned about keeping cases on track and minimizing the need for refinements.
- Patient Compliance Is Key
- Consistent wear is the number one predictor of success. Without it, planned movements won’t occur.
- Screen patients carefully before starting
- Set expectations early
- Encourage tools like TrayMinder app to track wear time
- Proper Fit & Seating
- Aligners move teeth in fractions of a millimeter. If they aren’t fully seated, no pressure is applied and movement will not happen.
- Pay extra attention at the start of each stage
- Stress the importance of using chewies (instructions included in every Active Aligners case)
- Ensure attachments are engaged correctly at every step, as they serve as anchors for movement
- Accurate IPR (Interproximal Reduction)
- Precision matters. Inadequate or uneven IPR is a common cause of refinement requests.
- Always use an IPR gauge (not strips) as measurement.
- Anticipate additional IPR later in treatment, as many plans require follow-up adjustments.
- Regular Monitoring & Tracking
- Even the best treatment plan needs close supervision
- Schedule a check-in one month after treatment begins
- Continue reviews every six weeks (or every 3 aligners)
- At each visit, check fit, attachment engagement, and progress against the 3D treatment plan
IMPORTANT: Mid-treatment restorative work should be avoided, as it can disrupt aligner fit and invalidate the plan.

- Communicate – Set realistic expectations and address discomfort
- Document – Record compliance, fit, and progress at each review
- Adjust Early – If off-track, extend wear time, backtrack to a prior aligners, add IPR, or re-bond attachments
